We are pleased to introduce our online census form.  The form was created to enable your family to quickly complete and submit the form online.  Please note the following:

1.   The form is an electronic duplicate of the paper form mailed to each parish family.  Similar to the paper form, every field can be filled electronically.  The results of your form are compiled and electronically sent to the parish office where they will be used to populate the parish database.

2.   The form is designed to be completed in one sitting. The functionality to save your work is not present within the form.  Therefore, please allow 20-30 minutes to complete the form. 

3.   We strongly recommend that you have all documents (marriage, communion and baptismal records, etc...) readily available before you begin.

4.   The form is an Adobe PDF document.  You will require Adobe Reader to complete the form.  It is likely that your computer already has Adobe Reader installed.  If not, you can download a free version of Reader by clicking here.
